My Student Needs Medicine At SchoolIf your student needs medicine while at school, you will need to fill out one or more of the following Medical Forms:
My Student Has...Asthma: Diabetes: Seizures:
MY STUDENT NEEDS TO TAKE MEDICATION DURING THE SCHOOL DAY
- Medication should not be transported on the bus by the student. Parent or Guardian over 18 must bring in medication.
- Medications should be brought in the original container to the clinic.
- Prescription medications must be labeled with the students name.
- Please check the expiration date prior to bringing to clinic.
- Please do not send medications in baggies with student.
- Please do not place medications in backpacks or lunch bags.
- No Tylenol, Motrin or Advil can be given unless it is provided by parent/guardian and a current authorization form is on file.

- Doctor’s Orders For Emergency Seizure Medication (including, but not limited to Diazepam, Diastat, Midazolam, and Versed)
